Indiana Youth Soccer Disclosure Form
This is a MANDATORY form that must be completed at least every 2 years. This is done to protect the players and anyone else involved in youth soccer. This is a statewide program that must be completed before anyone can be involved with the program in any position, whether coach, assistant coach, referee or any other volunteer including the Board. Please click on the link and do your part. If, for some reason, you are not comfortable with doing it online, print it off, fill it out and mail it to:
